In Utah, children must be 5 years old on or before September 1 to attend kindergarten. Please bring the following items with you, as they are required for students to attend school: birth certificate, immunization record, and proof of residence (utility bill).
If you live outside of Duchesne County or outside of the neighborhood school boundary, you will need to complete the open enrollment form.

The Walk-a-thon to celebrate America's 250th birthday is underway and students and staff are earning their laps. The prizes are starting to roll in so please support our efforts to purchase new bikes and helmets for our students by collecting those pledges and donations!!!! We are also accepting donations for raffle baskets, where the proceeds will go towards this effort as well.
